Real-World Impact Across Our Networks

Our financial year ended on 31st May and we took the opportunity to reflect on the significant progress we’ve made across 2024–2025. We’ve drawn together summaries from a range of our services that highlight the breadth of our work and the support we provide to Primary Care Networks and GP Practices throughout Wakefield.

Most importantly, these summaries demonstrate the real-world impact of our achievements, showing how our teams and partners are driving real improvements for patients across the Wakefield District.

We pride ourselves on our innovation and the dedication of our staff which enables us to make a positive difference for the people of Wakefield.

Our 2024 - 2025 Achievements

The NHS Health Check is for adults aged 40-74 in England to spot early signs health conditions. In the Wakefield District, practices deliver the majority of NHS Health Checks, and GP Care Wakefield provides additional checks on evenings & weekends.

Empowering 16–25 year olds through early, low-barrier mental health support.

In 2024/25, the Digital and Transformation team supported Wakefield GP practices with embedding digital innovations. The team provide greater efficiency, improved patient access, and frontline adoption of tools like the NHS App.

The MMR Catch-Up Project aimed to improve vaccine uptake in lower-achieving areas of Wakefield. Through targeted support to practices, staff training, outreach clinics, and improved referral pathways, the project boosted local uptake and strengthened collaboration across healthcare and community services.

Between July 2024 and March 2025, 4,498 children were supported by the Hub.

Our aim: To increase research activity in Wakefield by supporting more studies, engaging more GP practices, supporting more clinicians to participate, and recruiting more patients.

How: By providing practical infrastructure support to practices, building capacity in primary care, collaborating with system partners and promoting opportunities for practices and their patients to participate.

The Women’s Health Leads network was established to address the need for better access to healthcare for women throughout the Wakefield District.

The main goal was to develop the primary care offer and reduce the need for women to be referred to hospital services.

In 2024/25, Conexus provides a range of infrastructure support to our GP practices and Primary Care Networks by expanding both HR and finance services.

Established in 2024, created to offer a collaborative space for senior nursing leaders within our member GP practices.
