Strengthening General Practice Across the Wakefield District

Conexus Healthcare is the GP Federation for the Wakefield District, bringing together 34 GP practices that work collaboratively to improve local healthcare for our communities.

Our practices are grouped into seven Primary Care Networks (PCNs) allowing us to design services that truly reflect the health priorities of our communities. Together, we provide care for communities across Wakefield, Normanton, Pontefract, Featherstone, Castleford, Knottingley and beyond, ensuring every resident has access to trusted, accessible, and high-quality healthcare close to home.

Guided by our Five-Year Strategy (2023–2028), we’re investing in the future of primary care and empowering local GPs, improving patient experience, and strengthening community wellbeing across the district.
