Your Feedback Is Important

If you’ve used our clinical services, fill out our feedback form to tell us about your experience so that we know how we need to improve in the future. You can also use this form to pass on a compliment about something we have done well.

Complaints

If you are unhappy with our service, please make sure you speak to the team your problem relates to. We can usually rectify things quickly and easily. If you are unhappy with their response, then we can investigate further. This leaflet tells you what to expect if you wish to make a complaint.
